A head CT was ordered to further evaluate his persistent headache.

After about 68 weeks, the patient reported similar sensations in both feet. She also complained of perioral numbness and occasional urinary incontinence. She reported that she had been a strict vegan for the past 10 years prior to development of neurological symptoms. She reported she ate only vegetables, fruit, and bread. She never ate meat, poultry, fish, eggs, or dairy products. On physical examination, the patient had loss of proprioception and vibratory sensation at the T6-T8 level and had a spastic gait with abnormal cerebellar function. She was able to perform finger-to-nose movements bilaterally. Pertinent lab findings included a vitamin B-12 value of 190 pg/ml (normal 211911 pg/ml), folate of 20.6 (normal >5.6), homocystine of 59 umol/L (normal 4-14), and methylmalonic acid of 6.5 umol/L (normal <0.40). Hematological workup included a peripheral blood smear, which showed macro-ovalocytes.
